If you do not want to pay online with a credit card, you can complete this page and hit submit and send your payment to Diane Pemrick at the address in the brochure or print the registration page from the brochure and mail the payment to the address at the bottom.

If you are paying online, please complete the information below.  When you 'click' submit you will be taken to another page.  Click on 'Registration' and choose the type of registration needed and complete your payment online.  There will be a $5 credit card processing fee per registration.

**There will be a $50 non-refundable processing fee for cancellations made prior to 7 days before the meeting.  Any cancellations made within 7 days of the meeting are non-refundable.  Substitutions of attendee is allowed in either situation by notifying us at contactus@mnaaham.com.
